Chart Notes are also commonly referred to as:
- Medical Records
- Soap Notes
- Chart Notes must indicate why each lower level item will not medically suit your needs.
i.e.:
- If your prescription is for a walker it must be documented why any lower level item will not suit
your need such as a cane or crutch, OR
- If your prescription is for a lightweight wheelchair then there must be documentation that explains
why a standard wheelchair will not meet the medical need.
- Chart Notes must be from a visit to your physician about the requesting
equipment.
- Face-to-Face evaluation (if required for equipment)
- Prescription does NOT work as part of chart notes.
- The medical record is not limited to physician's office records but may include records
from hospitals, nursing facilities, home health agencies, other healthcare professionals, etc. (not all-inclusive).
- Records from suppliers or healthcare professionals with a financial interest in the claim
outcome are not considered sufficient by themselves for the purpose of determining that an item is reasonable and necessary.
