A Detailed Written Order is a document required by Medicare
that the DME supplier must have in order to bill for the requesting equipment. We MUST have the Detailed Written Order in our possession prior to the delivery of the equipment or
supplies.
Detailed Written Orders Contains ALL of The
Following:
- Beneficiary’s name
- Prescribing practitioner’s name
- The prescribing practitioner’s NPI
- Detailed description of the specific equipment that is to be provided
- Detailed description of each option/accessory that will be separately
billed
- The prescribing practitioner’s signature
- The date the prescribing practitioner signed the order (personally entered by
practitioner)
- The date of the order and the start date (if different from order date)
- The start date of the order is on or after a face-to-face (if required) encounter between the
prescribing practitioner and the beneficiary.
- The detailed written order was obtained prior to delivery of equipment.
- Any changes or corrections have been initialed/signed and dated by the prescribing
practitioner.
- A date stamp (or similar) clearly indicates the supplier’s date of
receipt.
- The practitioner’s signature on the written order meets CMS Signature
Requirements.
- Some Equipment requires the Detailed Written Order Prior to Delivery.
